public residential substance-use treatment and transitional housing program. Serves Adults experiencing chronic homelessness with alcohol-use and related behavioral-health needs. Intake: Program admission is coordinated through behavioral-health, hospital, law-enforcement and homeless-service referrals. Eligibility: Adults with qualifying alcohol-use and behavioral-health needs; program-specific clinical eligibility applies. Case management: Case management, recovery planning and supportive services. Mental-health services: Integrated behavioral-health and substance-use treatment. Transportation: Transportation support is part of the current program model.
