Intake: Direct referral from a contracted Los Angeles, Orange County or Inland Empire hospital or program; no public walk-in admission. Eligibility: Experiencing homelessness, recovering from illness or injury, medically appropriate for discharge from inpatient care, and referred by a contracted source. Documents: Clinical discharge and referral information supplied by the referring organization; confirm current requirements with Serenity. Published capacity: 22 adult beds in HUD’s January 2024 inventory and the County’s January 2024 stabilization invoice; not live vacancy. Waitlist/availability: Referring organizations coordinate current capacity; operator may place a client at another Serenity home if one location is full. Meals: Three hot meals daily; operator also describes nutritious snacks. Showers: Bathroom and shower access. Laundry: Housekeeping and laundry services. Case management: On-site case management and social-service connections. Housing navigation: Assistance identifying permanent-housing opportunities and building independent-living capacity. Medical services: On-site nurses, wellness checks, medication reminders and support following the medical plan of care. Mental-health services: Transportation and linkage to mental-health appointments; programs addressing mental health and substance use. Transportation: Transportation support to medical, mental-health and social-worker appointments, including staff transport, paratransit or bus passes as appropriate.
