Age: Adults; current operator does not publish a narrower age limit. Household eligibility: Individuals; current national directory also lists couples and families. Intake: Hospital, clinic or community-provider referral; individuals seeking services should visit the HOPE Van or primary-care provider. Eligibility: Experiencing homelessness and recovering from an acute or post-acute medical condition; clinical appropriateness assessed by the medical-respite team. Documents: Clinical referral worksheet and medical information. Accessibility: Hartman House was renovated to ADA standards; individual accommodation needs assessed during referral. Pets: Current national directory lists people with pets among populations served; confirm placement conditions. Service animals: Service animals handled under applicable law; program-specific procedures not published. Published capacity: 20 recuperative-care beds across fixed and scattered sites in the current certified national directory. Waitlist/availability: Referral team determines availability; operator documents an ongoing waitlist. Meals: Daily meals and supplemental food. Showers: Accessible showers at Hartman House. Laundry: Laundry provided. Case management: Medical and housing case management. Housing navigation: Housing-resource navigation. Medical services: Acute and post-acute medical care, wound care and coordination. Mental-health services: Behavioral-health and community referrals. Transportation: Transportation services. Donations: In-person donations accepted Monday-Friday 9:00 AM-4:00 PM; food, toiletries, clothing and other essentials requested.
