medical respite / recuperative care. Serves People experiencing homelessness or inadequate post-hospital housing who are medically and psychiatrically stable for discharge but need a safe place to recover. Household eligibility: Individuals. Intake: Hospital or healthcare-partner referral using the Illumination Health + Home referral form and supporting clinical records. Eligibility: Currently unhoused, at risk of homelessness, or in inadequate post-hospital housing; medically and psychiatrically stable; alert and oriented; able to manage activities of daily living, self-administer medication with oversight, ambulate 100 feet with or without equipment, and transfer independently. Documents: Referral form, demographics, authorization or letter of agreement, history and physical, current progress notes if needed, recent vitals, medication list, therapy or nursing notes, cultures if needed and 30-day medication supply. Accessibility: Medical-equipment use may be accepted when the client can ambulate and transfer independently. Published capacity: HUD 2024 recorded 85 adult-only beds. A 2025 planning application proposes conversion to a 144-bed center; that proposal is not treated as completed or current capacity.. Case management: Intensive care management and individualized recovery planning. Housing navigation: Housing navigation and connection to stable housing. Medical services: Post-hospital medical oversight and clinical support. Mental-health services: Behavioral-health services and substance-use counseling.
