Gender/access: Men, women, transgender and non-binary adults. Age: Adults, including older adults. Household eligibility: Individuals. Intake: Hospital, health-plan or other professional referral and admission review; contact the admissions/care-coordination team before discharge. Eligibility: Person experiencing homelessness or housing instability who is medically stable for discharge but needs recuperative support. Documents: Current medical history, medication list and other clinical documentation may be requested during referral review. Accessibility: ADA-accessible bathrooms and showers. Published capacity: Current national directory reports 47 beds for the Los Angeles program with two additional locations; HUD 2024 recorded 30 adult-only beds for the Vermont component. Neither figure is presented as real-time vacancy.. Meals: Three hot nutritious meals daily plus snacks. Showers: ADA-accessible bathrooms and showers. Laundry: Complimentary laundry. Case management: Care management, appointment coordination, benefits/documents and community-resource support. Housing navigation: Transitional assistance and support toward long-term housing. Medical services: Post-acute recovery oversight, medication support and follow-up appointment coordination. Transportation: Care-coordination support; confirm transportation during referral.
