Household eligibility: Individual patient referrals. Intake: A partnering healthcare facility or hospital sends the referral; a care manager evaluates needs and develops the care plan. Eligibility: Experiencing homelessness; medically stable enough to leave inpatient hospitalization but still needing a safe place and continued support to recover. Documents: Clinical referral materials required; use the operator referral process. Published capacity: 22 adult-only beds in HUD 2024; an older NIMRC profile lists 23, so confirm current licensed/operational capacity. Waitlist/availability: Operator publishes daily male, female and DME bed availability; call before referral. Meals: Meals provided. Laundry: Residential support; confirm specific laundry access. Case management: Care management and social services. Housing navigation: CES enrollment and linkage to temporary or permanent housing. Medical services: Medical oversight and medication management.
