long-term transitional residential program. Serves Young women aging out of foster care or facing homelessness or unstable housing. Gender/access: Female. Age: Ages 18-25. Household eligibility: Single and without children in their care; applicants may have children in another person’s custody. Intake: Submit online or printable admissions application; staff follows up within a few days. Eligibility: Female, 18-25, single, no children in applicant’s care, 60 days sober, able to pass drug screen, outpatient-manageable mental health, motivated for employment and independent living. Published capacity: Up to 29 women across program phases; Grace House accommodates 18. Waitlist/availability: More than 250 applications annually; current wait procedure not published. Meals: Food support and meal-planning education. Case management: Personal coaching, counseling, education, career guidance, life skills, and lifelong mentorship. Housing navigation: Preparation for independent housing and finances. Medical services: Medical care support. Mental-health services: Professional counseling; participant must manage needs with medication and outpatient support. Transportation: Transportation volunteer team and driver’s-license support. Donations: Financial, planned, food, household-item, gift-card, and in-kind donations accepted. Volunteering: Mentoring, transportation, meals, life-skills, farm, office, events, and maintenance roles.
